01 JUN 2004 Boeing 737 strikes antenna while landing at Brescia [to table of contents]
An Astraeus Boeing 737-700 (G-STRD) contacted an antenna just prior to touchdown at Brescia, Italy. The aircraft was inbound on a flight from London-Gatwick and sustained damage to one of the undercarriage legs. The landing was uneventful. (ANSV)

03 JUN 2004 BAe-146 damaged in heavy landing at Florence [to table of contents]
A BAe-146 (G-BPNT) suffered severe damage to the lower fuselage after touching down hard during a missed approach at Florence Airport, Italy. The airplane was on a ferry flight from Southend. (ANSV)

04 JUN 2004 Australian airlines subject to new safety checks [to table of contents]
Australia’s major airlines will be subject to special new safety checks by the Civil Aviation Safety Authority. The safety checks will focus in-depth on a key area of airline operations that has been identified as a potential risk. The first area to be subject to the new safety checks will be maintenance control systems within airlines. (CASA)

"The FAA proposes to assess civil penalties totaling $2.5 million against Atlantic Coast Airlines and United Airlines for allegedly violating Federal Aviation Regulations. The FAA alleges both airlines failed to follow required maintenance actions on specific aircraft, and further operated those aircraft when they were not in compliance with regulations." (FAA)

11 JUN 2004 Color vision deficiency factor in night-time CFIT crash [to table of contents]
The NTSB concluded their investigation into the July 2002 CFIT approach accident involving a FedEx Boeing 727 at Tallahasse. The captain and first officer failed to establish and maintain a proper glidepath during the night visual approach to landing. One of the contributing factors was the first officer’s color vision deficiency, which made it difficult for him to correctly identify the color of the PAPI signal. Two safety recommendations were made to the FAA regarding color vision test protocols. (NTSB)

25 JUN 2004 NTSB issues safety recommendations after Seattle taxiway landing events [to table of contents]
Three landings and three near-landings on Seattle (SEA) taxiway T (parallel to runway 16R) since 1999 prompted the NTSB to make three safety recommendations. One to allow SEA to use large nonstandard taxiway identification markings and two recommendations regarding research to establish marking standards for use on taxiways at airports with a recurring taxiway landing problem. 29 JUN 2004 Three injured in blast on THY 737 at Istanbul airport [to table of contents]
A package left on a THY Boeing 737-800 (TC-JFD) plane at the Istanbul airport exploded, injuring at least three aircraft workers. The plane was being serviced after arriving at Istanbul from a flight (TK317) from Izmir. (CNN, Milliyet)
